Right click on the form while editing the form in frontpage. You want to
go into the form properties page.

At the bottom you will see an advanced button. Click on it.
Click on the Add button to add name and value pairs
Name: To:
Value: email addy you want to send to

If not; you may have to reinstall the extensions but I would try this
first.
