Higuess we misunderstood manually - we did remove them through the interface but didn't check virtusertable to make sure they had gone.... this hasn't happened for every domain that we removed catch-alls from - just a few....
anyone know how safe to remove stuff from virtusertable above the custom line (or alter stuff) - and then just run

These should have been manually removed before installing OS
Upgrade 4.0. I can't say for sure, but you might have to remove them
from the /etc/virtusertable before you'll be able to add them again.
