Simple find a colo which offers 24/7 access.  Lease a direct line right into
their backbone connection and keep an eye on everything yourself.  You still
might hit bumps but the more you control on the bandwidth, the less problems
will occur.  Seems every company is bleeding their lines dry to make just
one more $.
