You haven't correctly setup reverse lookup for that ip
address. I.e. you have to add a PTR record to the right in-addr.arpa
zone.

The login failures of the Active Monitor are meant to be there. You
can ofcourse modify LogChecker so that it won't show up in it's
emails.
