i also had trouble with this.  i added an equivalent line to my rc.local
file as well (namely "/usr/local/sbin/portfwd -c
/usr/local/sbin/portfwd.cfg"), but it didn't seem to work as
planned...at first.  some time later (weeks?  months?) i noticed that
the portfwd utility did auto start on reboot.  i have no idea why it
didn't work at first but later decided to.  such is life i suppose.

in any case, just for the fun of it i went ahead and wrote a little init
script for portfwd, which i placed with all the other start/stop scripts
in "/etc/rc.d/init.d".  i then changed the line in rc.local to read
"/etc/rc.d/init.d/portfwd start portfwd.cfg".  i would be happy to send
you (or anyone else for that matter) the init script i wrote if you
think you would find it useful, but keep in mind that i'm just a hack
and don't really know what the hell i'm doing.

it all seems to be working ok now, but then again it has been awhile
since i have rebooted, so i may not really know what i am talking about
(it happens from time to time donchaknow).
