Check out cgi.resourceindex.com and look for alienform under perl->form
processing.
That will do for the personalized reply from a text file template and
multiple paged forms. The tracking number you could probably make by
sticking a javascript in that takes the time that the form is downloaded
to the browser and makes that, after some modification, the tracking
number. I was doing that with alien form myself, though we decided that
the client didn't need something so sophisticated just yet...
