There is a multi-page form that can be used for this, written in PHP though
(not CGI). It's Form Processor. If you go to http://php.resourceindex.com
you'll find it.
I also found a support ticket script at CGI Resources
(http://www.cgi-resources.com) that didn't work worth a damn but gave me a
good outlay of ideas; I rebuilt the thing from the ground up and got it
working. Just do a search at CGI Resources for 'support ticket' and it
should pop up. You might get it to work right off, too... who knows, maybe
it was my RaQ.
