We allow our dialup ISP users to choose their own passwords; so many
dialup ISPs do that we find ourselves at a competitive disadvantage if
we don't.

For our hosting packages we've got a DOS-based program that returns a
password such as: KNv4ywQh
(don't worry; we've never used this one before, and now we won't ever
use it).
