Thanks for the help.
I have some more info.

The error in the log is this.
"malformed header from script. Bad header= info@xxxxxxxxxxxx$

Do you have any ideas on what this error is saying?

Now about these other questions.

YES! The client can retrieve other mail. It is just the form that wont work.

It works with no problem on other mail addresses off the server.
