Often occurs on high-volume lists when outgoing messages are still
sitting in the queue (because they haven't been sent to all addresses
yet) when a second daemon starts (usually ten or fifteen minutes later,
depending on configuration on the MTA) starts to process the queue.
Can be fixed (pretty-much) when using majordomo, by going through
"bulk_mailer". We've seen this problem quite a bit on our list-hosting
business.

Yup; this happens from time to time as well; the mta at webexpress24.com
is misconfigured to send failures to the "from:" address rather than to
the envelope from address.
