How long after adding the extensions did you wait until you tried to publish
using FrontPage? Sometimes you just wait a while and suddenly FrontPage
starts working. I'm not sure why.

Are you trying a site administrator for that domain as the user name and
password?

yourself... any old page and see if you get the username/password challenge.
Did you put any .htaccess file in the directory or make any change to your
httpd.conf file?
