Since it's one of those things that has absolutely no effect on anyone
outside of your server, I don't think there would be, or needs to be, a
standard. index.html/htm has become an accepted standard, (makes it easy for
customers to not have to change all their files when they change ISPs), but
there is no RFC that I've ever heard of on this.
