I have had that problem. I first thought of DNS as well, but it wasn't in my case. Somehow my virtusertable got corrupted. Try restoring a backup of your /etc/virtusertable and then issuing MakeMap. That ought to do it. Be sure to make a copy of your current virtusertable so you can go back to it if need be.
