You need to make sure that your message starts with 550 as this is the error
code for the sending SMTP server telling it that this is a permanent failure
and the mail should be bounced.
If result code 550 is missing, the sending server will Q the mail and keep
nagging your server to accept it till it expires :)

You can put whatever you want in there.
I believe you're limited to 80 characters according to the RFC's for
rejection messages. Personally, I give a URL they can visit since some
IPs are rejected because they're dynamic or DHCP blocks so I don't say
it's spam in all of the messages.
After you make the change to Sendmail.cf you need to restart Sendmail.
