Your system limits are not suitable for the load you have. Increase
them.

Broken page errors. Ignore - or tell your users not to surf URLs as
broken as that.

You restarted while there still was a previous cache process running.

Broken page errors again. Inevitable, as long as no web police patrols
and shuts down broken sites...
