The question is "per server", which may mean single IP#s, range or class
of IP#s, or even multiple IP#s from separate classes.

It's a common question; it's a common need of anyone who does
colocation... how do we keep our clients from using for bandwidth than
they've paid for?

Or at least, how can we be notified when they're attempting to use too
much bandwidth, so we can let them know, or do something manually.

The only way I know how to do it is with (expensive) router options.
Anyone have a good idea?

We have an appliance that is 3K that is specifically designed for ISPs. Its also available as a software add-on for linux or freebsd for $695. You can manage by MAC, IP, protocol, port , virtual host and will even sniff out PTP. At one time it ran on a raq3i but they were just too underpowered and overpriced at the time.
