I can't say that I've ever had this problem, but I have email configured to
ONLY relay from machines in my LAN. If you have remote users you can enable
POP before SMTP, but I prefer to just tell them to relay through whatever
ISP they are connected to.

It was my understanding that sendmail on a Qube3 was configured to only
accept email terminating on the Qube3 or being sent from implicitly allowed
address.

Having said that I have heard that the implementation of caching used on the
Qube3 is vulnerable to being used externally as a relay ... this could
actually be your problem.

Try disabling the Web Cache capability.

You could also installing a hardware firewall device between your Qube3 and
the internet.

Cheers, Malcolm
