Assuming the code for Neomail is fairly well written an experienced Perl web programmer should be able to make either modification in under an hour. If you actually had Neomail physically installed on each site you could just use an .htaccess file to accomplish alternative 2, but that would be pretty cumbersome. I gave up serious Perl programming 3 years ago so I'm afraid I can't be of further assistance.
