Take a look in the users archive for duplicate entries in your
virtusertable. I had that problem once and that seemed to play a role.
Search on the term 'makemap hash', it should get you in the right
neighborhood.

PS - Your lucky day, I found a note I saved on my computer:

makemap hash /etc/mail/virtusertable < /etc/mail/virtusertable
