Unfortunately not.  You did spur me to double check it though, and
the case and extension is correct.

Has anyone else had this issue before?

One of our users, using FP (on a RAQ3i) had a similar issue, the
whole 
problem was her FP was setup to create Index.htm instead of
index.html - 
case and extension were wrong. I assume your problem is not something
this 
simple  ??
