There is MRTG which monitors SNMP data from your network and 
can do some accounting, depending on your network setup. The
best one out there is packetshaper. it isn't a piece of software
but it is hardware that costs about 20k.
It will do everything you could want. Accounting down to the 
webpage and the content type of it. You can do QOS and service
level guarentees.
